      The Enrolment Form

      This session requires you to fill in your personal information and complete 3 forms before you log onto the IVET Portal.

      Depending on the setup of your school, you may be required to complete 3 forms includes: an enrolment form, a pre-training review and an LLN assessment the first time you log into the IVET Portal.

      Part 1: Fill in your personal information

      Firstly, click on Enrol Now as picture below:

      Next, this section will require you to review again and confirm your Personal Information before enrolling to the course.

       OR should you had already enrolled in several courses before, please make sure that your Personal Information is still correct to continue with next course.

      If you finish confirmation with your Personal Information, please click "Enrol" button on the right hand side to move on "Complete Forms" part. 

      Part 2: Complete forms includes: The enrolment form, The Pre-Training Review Form and The LLN Assessment.

      These forms must be completed before you can access your class. Please allow approximately 30-50 minutes to complete these forms. You can find further details on what each of these forms entails in the sections below.

      These forms will ask you to complete the following details: 

      • Full Legal Name 
      • Unique Student Identifier (USI) 
      • Date of Birth 
      • Contact details: phone number, address and email address 
      • Language and cultural diversity information 
      • Disability, impairment, or long-term conditions 
      • Education history 
      • Reason for study 

      The Enrolment Form 

      Our enrolment form is used to assist us in collecting and submitting data compliant with AVETMISS for the National VET Provider Collection as required by the Data Provision Requirements 2012 and National VET Data Policy.

      In order to supply you with a certificate upon completion of your course, and to ensure your USI records are updated, IVET requires the information entered in this enrolment form.

      Please note: As from the 1st of January 2015, all students in Australia enrolling in a VET course require a Unique Student Identifier (USI) number for the Australia Government Department of Industry. Without this number IVET is unable to provide you with a VET qualification or a VET statement of attainment. If you do not have a USI number, use the following government link to create your own USI number: https://www.usi.gov.au/students/create-your-usi
